Youth Leader Report (YLR)

1. Where do I get this?
   Please complete this YOUTH LEADER APPLICATION FORM & Payment.

   ---

   *The court system requires your Social Security Number and Driver’s License Number to process the background check, but those items cannot be collected with this payment for security reasons.  You may receive a call from one of the Applied Human Sciences staff to collect those secure items.  You may ask the caller to provide a password that you have chosen on the application so you can feel secure in providing the information.  

    

2. How much does it cost?
   $25 payable by credit or debit card is due with your application at the TouchNet site above.
    
3. How long is this valid?
   It is valid for 1 year. 
    
4. If I had this report done somewhere else, can I use that?
   NO-We cannot accept a report completed by any outside agency.  
    
5. What If I have a criminal record?
   You will be contacted by the Department Chair's office through our EKU email.  Please check your email often.
